– First, go to FoxChiropracticClinic.com. At the top of the main page, click on the NEW PATIENT dropdown and then to Online Forms. Please fill out the online or printable form as completely as possible. You can submit the online form electronically or you can bring the printable form with you to your initial visit.
– Make sure you have your drivers license or a form of photo ID with you when you arrive.
– If you have insurance, please bring in your card for us to copy.
– A list of medications or special needs will also be helpful, if applicable.
