The New Patient Health History Form will let us know more about your health history and your current condition and health history.
To save time on your first visit, please click on the “Online Form” hyperlink below and fill it out as complete as possible. Then submit it electronically. Detailed questions about your health will be addressed by Dr. Fox.
A second option is to download, print, and fill out the form. You can then either bring the form with you, fax it to us at (785) 456-8882, or email it to us at foxchiropractic@msn.com.
